Fresh Food Farmacy

2024 will be CFC’s sixth season of the Fresh Food Farmacy Program in partnership with Hudson Headwaters Health Network (HHHN) at four health centers in the Southern Adirondack region. The Fresh Food Farmacy provides qualifying patients with dietary education and medically-tailored food boxes for a 24-week program. We also host Pop-Up Farm Stands to distribute bags of local produce, recipes, and food resources during the peak growing season at participating health centers.​

Produce Prescription Programs

Comfort Food Community partners with local healthcare organizations to bring vouchers for local, fresh food to our community members. We have two Produce Prescription Programs: one through our partners at CDPHP and one through our partners at Glens Falls Hospital. Click to learn more about each program and find out if you qualify!

Here at CFC, food access is about more than just food! Having the knowledge and tools necessary to prepare nutritious and yummy meals is crucial. Here are some of our main programs aimed toward giving those tools to our community members:

  • Healthy Meal Kits complete with ingredients, recipes, and nutritional information provided to guests at our Food Center. To date, 800+ meal kits have been distributed at our pantry!

  • Community Workshops hosted by partner agencies at our Food Center on topics such as Medicare enrollment and Smoking Cessation. 

  • Food Demonstrations and workshops highlighting featured food items. In 2023, off-site workshops were hosted at Queensbury Senior Center, ASCEND Mental Wellness, Greenwich Free Library, and more!

Food Eduaton

 “I am diabetic and a single mother on a very limited budget and having access to local farm’s food has been beneficial to my health. I was able to get foods that I would normally not buy or aren’t available in my market. I also started a new medication when I joined the Produce Prescription Program, which allowed me to start eating more vegetables, fruits, fish, and yogurt/dairy. This also helped me cook more vegetables and fish. My sugars are almost within normal range, which is great because they were dangerously high earlier this year. My blood sugars are now under good control.

Since meeting CFC staff and being a part of Comfort Food Community’s Glens Falls Farmers Market Produce Prescription Program, I'm eating better. I am more aware of what I need to eat in order to stay healthy not only with my diabetes, but also with anxiety, inflammation, and other health issues. She sent me information to help me understand the nutritional impact of food on my mind and body. She also guided me in what types of food to eat to help ease anxiety. Knowing the foods I receive from the farmers market are healthy and fresh, I believe they are the best for my son and me. I am thankful for this program and appreciate all that CFC has done! Resource Navigation

The Community Health Team accepts incoming referrals from providers as well as self-referred individuals in the region to help connect people to food assistance services and other services beyond emergency food. Our toolbox includes a regional referrals platform, as well as direct knowledge of enrollment processes for benefits, assistance with housing searches, and advocacy at Social Services.
