The purpose of Comfort Food Community’s produce prescription programming is to improve health outcomes related to chronic illness and food/nutrition insecurity by connecting eligible participants with vouchers to spend on fresh, nutritious foods by local producers, and by providing education related to nutrition, skills building, and resource sharing.

What
A 5-month nutrition incentive and education program intended
to connect participants with fresh, locally sourced produce and nutrition education. Participants receive five, $5 vouchers ($25) in the mail five times for a total of $125.
When
Enrollment begins in April 2025 and is open while supplies last. $25 in vouchers once per month to spend on produce, dairy, meats, and other eligible items from participating vendors throughout Washington County.
Who
Glens Falls Hospital is the primary screening and referral partner for this program. While the focus is on Medicaid, Medicare, and Essential Plan members, insurance type and provider are not current eligibility factors for this program. Eligibility is determined by screening positively for food insecurity and identifying the presence or risk for a chronic health condition.
How
Designated partners in Glens Falls Hospital care management program, clinical nutrition programs, and cancer center program will screen and refer patients to CFC for enrollment in the program.

Comfort Food Community Enhanced Service
Produce Prescription Program
In 2024, New York State applied for and was awarded an 1115 Waiver under state Medicaid to create a statewide program for screening and enrolling eligible Medicaid members in “enhanced services” that support a variety of health-related social needs. The NYS waiver established Healthy Alliance as the lead entity for the Capital District and North Country. Healthy Alliance is in the process of building a regional Social Care Network of contracted service providers that will offer “enhanced services” to eligible Medicaid members. Individuals must be screened and referred to enhanced services by Healthy Alliance-affiliated resource navigators (utilizing the secure Unite Us platform). As part of the Social Care Network lead by Healthy Alliance, CFC will be offering an “enhanced service” version of the Produce Prescription program.
What
For up to 6 months, eligible participants can sign up to receive a weekly packet of vouchers by mail. The total weekly $ amount of vouchers (yet to be determined) will be sufficient to provide 3 meals a day, 7 days a week per participant.
Monthly voucher packets will also contain recipes, nutrition information, and additional resources and programs related to food access and other health-related social needs.

Enrollment for this program will be year-round and open as supplies last.
Who
Current Medicaid enrollment is a requirement for this program. There are additional criteria that must be met for an individual to be eligible for enhanced services. Individuals who are interested must be screened by a Healthy Alliance-affiliated screener.
Vouchers can be spent on eligible produce, protein, dairy, grain, and healthy value-added items at participating farmers’ markets, farm stands, and local grocery stores/co-ops in Washington County and some locations in Saratoga and Warren counties.

Produce Prescription Program
What
A 5-month nutrition incentive and education program intended
to connect participants with fresh, locally sourced produce and nutrition education. Eligible CDPHP members may sign up to receive five, $5.00 vouchers ($25.00) up to FOUR times (for a total of $100 worth of vouchers).
When
This program runs from June-December. Vouchers are available as supplies last and expire at the end of the year. Participants must visit the CFC table at participating farmers’ markets to receive market vouchers. See the schedule below for Famers Market dates and locations.
Who
Participating markets are Glens Falls Farmers Market, Saratoga Farmers Market, Canal St. Marketplace in Fort Edward, and Cambridge Valley Farmers’ Market. Eligible insurance types may change from year to year. This year, eligible insurance types include Medicaid/HARP, Medicare, Essential Plan, and Child Health Plus.
How
CDPHP will send eligible participants in Washington County (and some zip codes in Saratoga and Warren counties) an invitation in the mail beginning in May to enroll in the program. Members may also be screened by a CFC staff person at the farmers’ market for eligibility and enrollment. Enrollment is open as supplies last.
