About this opportunity
In partnership with Get Healthy North Country and Comfort Food Community, Washington County Public Health is pleased to offer a FREE 6-week Healthy Living: Pathways to Positive Change Workshop for Washington County residents. The workshop is an SMRC Evidence-Based Self-Management Program originally developed at Stanford University and is recommended for adults with at least one chronic health condition and/or their family members, friends, or caretakers. Chronic diseases may include but are not limited to arthritis, lung problems/diseases, cancer, stroke, hypertension, anxiety/depression, obesity, heart disease, and tobacco-use-related conditions.
Participants will learn new skills to help manage chronic diseases like how to cope with stress, pain, and fatigue, setting realistic goals, and more.
