Are you passionate about building healthier communities through food? Do you believe food is more than just fuel—it’s medicine? Comfort Food Community (CFC) is seeking a full-time Food as Medicine Coordinator to help bridge the gap between healthcare and healthy food access.

As part of our Community Health Team, you’ll lead the implementation of innovative Food as Medicine programs in partnership with regional healthcare providers—connecting patients with nutritious food, education, and vital community resources.

What You’ll Do:

  • Coordinate logistics for the Fresh Food Farmacy program, including food purchasing, packing, and delivery of medically tailored produce boxes

  • Manage Produce Prescription (PRx) program operations, including referral intake, voucher distribution, and vendor communication

  • Track and evaluate program data, budgets, and performance metrics

  • Build and maintain relationships with healthcare providers, farmers, vendors, and community partners across Saratoga, Warren, and Washington Counties

  • Represent CFC and promote PRx at community events and farmers’ markets

  • Support ongoing Community Health initiatives, pantry services, and cross-program collaboration

  • Stay up to date with HIPAA and data privacy protocols

What You Bring:

  • Bachelor’s degree in public health, social work, nutrition, or a related field (preferred)

  • 1–3 years of experience in public health, food systems, or outreach (internships count!)

  • Strong communication, presentation, and relationship-building skills

  • Experience working with diverse communities and populations with limited access to healthy food

  • Detail-oriented with experience in data tracking and program coordination

  • Commitment to health equity, food justice, and community-based solutions

  • Proficiency in tools like Microsoft Office, Teams, and Canva

  • Familiarity with local food systems and regional resources

Physical Requirements & Work Environment:

This position includes some physical tasks like lifting up to 50 lbs, working in coolers/freezers or all-weather environments, and driving larger vehicles. You should be comfortable using manual/power pallet jacks. Occasional evening or weekend work may be needed for outreach or program logistics.

Why Work With Us?

At Comfort Food Community, we’re redefining the role food plays in wellness and recovery. You’ll be joining a passionate, collaborative team that believes in community-led, data-informed programming—and the healing power of good food.
